On August 21, 1995, Kenneth Michael Trentadue, an inmate at the Federal Transfer Center (FTC) in Oklahoma city, was found dead, hanging by a bed sheet around his neck from a vent grate above the sink in his cell. An investigation by the Office of the Inspector General (OIG) found that Trentadue's death was a suicide, that he had not been beaten and murdered, and that the Bureau of Prisons (BOP) did not participate in any conspiracy to cover up his death. But, the OIG did find that FTC employees made various mistakes in their response to Trentadue's death. This summary presents the major findings of this investigation.
