Serious Incident Review: Connally Unit: December 13, 2000

ANNOTATION: Around 1120 hours on December 13, 2000, seven inmates assigned to the maximum-security Connally Unit escaped. This review examines this unit's security operations and procedures, maintenance procedures, emergency response plan, unit staffing, and the unit classification process in order to identify existing problems and to offer recommendations and corrective actions to prevent a similar event occurring in the future. While the escape was very well planned, non-compliance in the areas of security operations and procedures and maintenance procedures contributed to the success of the escape.
