ANNOTATION: A best practice transition planning model for offenders with co-occurring disorders released from jails is presented. Sections of this publication include: abstract; introduction; the APIC Model -- assess, plan, identify, and coordinate; and conclusion. These elements should improve outcomes for this special population, even though which "elements are most predictive of improved outcomes awaits empirical investigation" (p. 18).
