ANNOTATION: The development of an assessment-driven recovery plan tailored to each offenders needs, strengths, and aspirations is described (p. 1). Sections of this report are: executive summary; system overview; Phase Iinstitutionally based programs and preparation for release and discharge planning during incarceration; Phase II--transitional services; Phase III-achieving stability in the community (with or without formal community supervision); community education; quality assurance; and policy and legislative issues for consideration. Six sections comprise the Action Plan: introduction; pre-release planning, transition, and case management action items; education/employment action items; housing action items; public education and support action items; and legislative and policy action items.
