ANNOTATION: An investigation of an escape from the Iowa State Penitentiary (ISP) on November 14, 2005 is presented. Sections comprising this report are: executive summary; background of the Iowa State Penitentiary; background of the offenders that escaped; escape timeline; escape planning and materials; findings related to tool control and inventory, offender counts, offender searches, post orders and policies, security coverage, reporting of critical incidents, and classification of offenders; Department of Corrections' response; Executive Branch response; and conclusion. This escape was facilitated by a breakdown of security.
