ANNOTATION: This chart provides an overview of the Sequential Intercept Model, "a conceptual framework for communities to organize targeted strategies for justice-involved individuals with serious mental illness" (p. 1). The model utilizes five intercepts -- law enforcement, initial detention/initial court hearings, jails and courts, reentry, and community corrections. Action for system-level change, action steps for service-level change at each intercept, and three major responses for every community are also reviewed.
