Webinar Summary: 

Both CDR Natalie Li and LT Colby Kern, Advanced Practice Diabetes Pharmacists have robust comprehensive diabetes clinical care practices. They independently follow the most complicated diabetes patients at their institutions. By providing education and therapy adjustments, they assist provider teams in helping these difficult patients strive towards their treatment goals with an emphasis on reducing complications and patient safety. From their experiences they will share clinical examples of patient care for patients with type 2 diabetes, emphasizing the impact of the clinical pharmacist on diabetes care to adults in custody.

During this 60-minute webinar, participants will:

  1. Identify team-based strategies for reducing complications of type 2 diabetes and associated co-morbidities in carceral settings.
  2. Evaluate multi-target pharmacotherapy approaches for type 2 diabetes, cardiovascular disease, heart failure, obesity, and kidney disease.
  3. Apply guidelines to patient-specific scenarios to optimizing therapy and reduce complications within the context of the secure environment.

  • LT Colby Kern

    LT Colby Kern

    LT Colby Kern is a staff pharmacist at FCC Butner in Butner, NC. She began her career with USPHS as a SRCOSTEP participant and graduated from Sullivan University College of Pharmacy in 2020. She has served at FCC Butner since 2020 and is a lead pharmacist in the comprehensive diabetes management clinic.

     LCDR Li

    CDR Li is an advanced practice pharmacist at the Federal Medical Center in Lexington, Kentucky. Her time is dedicated to skillfully managing a comprehensive clinic with a primary focus on diabetes, weight management, hyperlipidemia, and medication therapy management. Additionally, she plays an instrumental role in running the "Managing Your Diabetes" course, empowering patients to take charge of their health. LCDR Li holds board certification in Pharmacotherapy and has earned certification as a National Clinical Pharmacy Specialist, through her outstanding contributions to clinical pharmacy services. In 2022, she was recognized as the BOP New Clinical Pharmacist of the Year, further highlighting her exceptional dedication to advancing patient care. Prior to her work at the BOP, LCDR Li worked in the Indian Health Service at Alaska Native Medical Center in Anchorage, Alaska. 

    During her tenure, she excelled in providing an array of clinical services, spanning inpatient, emergency, ambulatory, specialty, transitions of care, and telehealth pharmacy. LCDR Li completed her PGY1 Ambulatory Care Residency at Southcentral Foundation in Anchorage, Alaska. This experience enabled her to develop a strong foundation of clinical skills, particularly in patient education, tobacco cessation, diabetes management, mental health support, and Medication-Assisted Treatment (MAT). LCDR Li earned her PharmD degree from University of Kentucky in 2016.
